As a Care Coordinator at Anglicare, you’ll play a role in supporting older Australians to live independently in their own homes. You will support the coordination of client services, ensuring they are aligned with client individual's goals, wellbeing, and preferences. Working closely with internal teams, clients, families and community networks, you’ll be ensuring high-quality, person-centred care.

Job Type: Full-time (6 month contract)
Key Responsibilities
Quality & Compliance – Ensure services are delivered in accordance with the Aged Care Quality Standards, Charter of Aged Care Rights, and relevant program manuals (e.g., HCP, CHSP).
Documentation & Reporting – Maintain accurate and timely records, case notes, and client assessments in our digital systems.
Stakeholder Engagement – Build strong relationships with clients, families, carers, and internal teams to support holistic care planning and delivery.
Team Support & Feedback Management – Support the care team by capturing and logging client and service feedback through the RaiseIt system, contributing to continuous improvement.
Problem Solving – Respond to changes in client needs, escalate issues appropriately, and ensure timely service adjustments.
Continuous Improvement – Contribute to service enhancements, quality initiatives, and collaborative team discussions that improve client experience and outcomes.
What We’re Looking For
Certificate III or higher in Aged Care, Community Services, or equivalent
Previous experience in community-based aged care or home care coordination
Strong communication skills with the ability to build rapport and work collaboratively with clients and teams
High level of organisation, initiative, and attention to detail
Intermediate computer literacy and ability to work with electronic client records
A compassionate approach and alignment with Anglicare’s Vision, Mission, and Values
Knowledge of aged care funding (HCP, CHSP) and compliance frameworks is desirable
